About the role

Key responsibilities & impact

Provide long-term care management to older and disabled adults
Conduct in home and telephonic assessments, care planning, and client monitoring
Coordinate linkages with community-based services and resources
Maintain legal and clinical written records of each client’s case
Ensure timely implementation of services and client needs
Maintain effective communication with other disciplines
Increase knowledge of care planning functions

Requirements

What you’ll need

Bachelor's Degree in Social Work, Psychology, Gerontology or related field required
BILINGUAL- Must be bilingual in English/Korean
1-2 years experience working with the elderly
1+ year experience in a hospital, home health agency or related setting for older adults
Ability to travel within program’s catchment area
Knowledge of environmental concerns of older adults
Strong organizational, verbal and written communication skills
Experience working in a case management setting preferred
