About the role

Key responsibilities & impact

Deliver compassionate, patient-centered care in a hospice setting
Collaborate with an interdisciplinary team dedicated to comfort, dignity, and quality of life
Conduct comprehensive psychosocial and bereavement assessments
Identify patient and family needs and develop supportive care plans
Provide counseling, emotional support, and guidance to patients and families
Connect patients and families with resources and help them navigate end-of-life care
Address complex situations and advocate for patient wishes
Ensure accurate, timely, and compliant EMR documentation
Work independently while collaborating with the care team
Travel to patient homes within the assigned territory

Requirements

What you’ll need

Master’s in Social Work from graduate school accredited by the Council of Social Work
One (1) to two (2) years post graduate social work experience in a health care setting
Must hold, in good standing, a license as a Medical Social Worker issued by the state in which you work
Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
Successful completion of a drug screen prior to employment
Legally authorized to work in the United States
