About the role
Key responsibilities & impact- Deliver compassionate, patient-centered care in a hospice setting
- Collaborate with an interdisciplinary team dedicated to comfort, dignity, and quality of life
- Conduct comprehensive psychosocial and bereavement assessments
- Identify patient and family needs and develop supportive care plans
- Provide counseling, emotional support, and guidance to patients and families
- Connect patients and families with resources and help them navigate end-of-life care
- Address complex situations and advocate for patient wishes
- Ensure accurate, timely, and compliant EMR documentation
- Work independently while collaborating with the care team
- Travel to patient homes within the assigned territory
Requirements
What you’ll need- Master’s in Social Work from graduate school accredited by the Council of Social Work
- One (1) to two (2) years post graduate social work experience in a health care setting
- Must hold, in good standing, a license as a Medical Social Worker issued by the state in which you work
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
- Successful completion of a drug screen prior to employment
- Legally authorized to work in the United States
