About the role
Key responsibilities & impact- Provide end-of-life care to patients in their homes in the Staunton, IL area
- Deliver compassionate, patient-centered hospice care
- Collaborate with an interdisciplinary team focused on comfort, dignity, and quality of life
- Perform comprehensive patient assessments
- Manage symptoms and pain
- Develop individualized end-of-life care plans
- Educate and support patients and families through the end-of-life journey
- Think critically and solve problems in real time
- Maintain thorough, compliant EMR documentation
- Work independently while traveling to patient homes within the assigned territory
Requirements
What you’ll need- Active Registered Nurse (RN) license in the state you work
- Two years of RN experience
- Clinical expertise to deliver quality patient care
- Hospice experience preferred
- Current driver’s license
- Ability to spend approximately 20% of the day driving to and from patient locations
- Successful completion of a drug screen prior to employment
- Legally authorized to work in the United States
