About the role
Key responsibilities & impact- Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
- Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
- Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
- Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR
- Work independently, but never alone
- Travel to patient homes within assigned territory
Requirements
What you’ll need- Active Certified Nursing Assistant (CNA) certificate in the state you work
- High School Diploma or certificate
- Minimum one year healthcare experience
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
