About the role
Key responsibilities & impact- Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
- Perform comprehensive patient evaluations focused on activities of daily living (ADLs), upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life
- Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines
- Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
- Work independently, but never alone
- Travel to patient homes within assigned territory
Requirements
What you’ll need- Active Occupational Therapist, OT license in the state you work
- One year of experience as an Occupational Therapist and the clinical expertise to deliver quality patient care
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
