About the role

Key responsibilities & impact
  • Complete Comprehensive Assessment and Comprehensive Assessment Survey
  • Complete GAIN-SS with all clients upon admission
  • Complete Initial Services Plan with all clients
  • Contact referents within the first three days of admission and weekly thereafter
  • Assess clients for potential vulnerabilities using criteria stipulated in the Program Abuse Prevention Plan
  • Develop comprehensive individualized treatment plans with specific and measurable goals, objectives and interventions
  • Document client participation and progress in client record
  • Communicate regularly with Outpatient Clinic Director and Mental Health Professional on the treatment and progress of clients
  • Facilitate group chemical dependency counseling and education sessions

Requirements

What you’ll need
  • Full licensure by the State of Minnesota for Alcohol and Drug Counselor in good standing
  • Know and understand the implications of chapter 245G, chapter 245A, and sections 626.556, 626.557 and 626.5572
  • Constantly operate a computer and other office machinery, such as a calculator, copy machine, printer, etc
  • Frequently communicate with employees, managers, patients and other individuals with key relationships
