About the role
Key responsibilities & impact- Conduct in-home client assessments

Requirements
What you’ll need- Active Registered Nurse (RN) license in the State of New Jersey (Required)
- 1+ years of Nursing experience, preferably in home care, community health, or case management
- Current BLS/CPR certification (Required)
- ACLS and/or PALS certification (Preferred)
- Bachelor of Science in Nursing (BSN) preferred; MSN is a plus
- Experience with clinical assessments, care plan development, and patient education
- Valid NJ Driver’s License, reliable transportation, and ability to travel locally for in-home visits; must meet required medical and background clearance guidelines
