About the role
Key responsibilities & impact- Assess client needs
- Develop and update individualized Plans of Care
- Provide skilled nursing services
- Help ensure that care meets professional, regulatory, and agency standards
- Supervise and support Certified Home Health Aides
- Collaborate with physicians, therapists, caregivers, and other healthcare professionals to provide coordinated care
- Conduct initial and ongoing in-home nursing assessments
- Provide direct nursing care based on client needs
- Monitor clients and identify changes in condition
- Communicate significant findings to families, physicians, and the care team
- Complete accurate and timely clinical documentation
- Coordinate services with healthcare professionals and caregivers
- Conduct supervisory visits and evaluate care provided by home health aides
- Provide education and clinical guidance to caregivers and families
- Follow infection-control, safety, HIPAA, and agency requirements
- Report incidents, concerns, and changes in client condition
Requirements
What you’ll need- Active New Jersey Registered Nurse license - In good standing
- Current CPR certification
- Strong clinical assessment and critical-thinking skills
- Home care, home health, geriatric, rehabilitation, or community nursing experience preferred
- Experience supervising Certified Home Health Aides preferred
- Knowledge of medication management, infection control, documentation, and care planning
- Current negative TB test or chest X-ray
- Ability to complete all required background checks and screenings
- Excellent communication, organization, and customer-service skills
- Compassionate, dependable, professional, and detail-oriented
- Ability to work independently
- Comfortable using technology for scheduling and clinical documentation
- Ability to travel throughout Bergen County
- Valid driver’s license and reliable transportation preferred
