About the role
Key responsibilities & impact- Evaluate patients in their homes and develop individualized treatment plans
- Provide skilled OT services to improve daily living skills and functional independence
- Educate patients and caregivers on adaptive techniques and safety
- Document evaluations, progress notes, and discharge summaries in a timely manner
- Collaborate with interdisciplinary healthcare team members
- Follow all state, federal, and agency regulations
Requirements
What you’ll need- Current Occupational Therapist license in good standing
- Graduate of an accredited OT program
- Home health experience preferred (but not required)
- Reliable transportation and valid driver’s license
- Strong communication and time-management skills
- Ability to work independently
