About the role

  • Implementing evidence based programs in the Hispanic community with emphasis in nutrition, physical activity and chronic disease management.
  • Provide support through advocacy services, care management and implementation of programs that promote healthy lifestyles for children, families and seniors.
  • Providing support to child safety car seat programs,
  • Living Well with Chronic Conditions (“Tomando” and “Manejo” Stanford evidence based programs,)
  • Facilitating group health presentations
  • Become an advance care planning facilitator
  • Implement nutrition and physical activity initiatives for all age ranges

Requirements

  • Bachelor’s degree or equivalent experience in health promotion, community health education, social work, nutrition, exercise science or related field
  • Broad knowledge in nutrition, physical activity and chronic disease management, including strategies and best practices.
  • Certified fitness instructor and/or nutrition educator a plus.
  • ”Tomando Control de su Salud,” “Manejo de la diabetes” and/or National Diabetes Prevention Program leader certifications a plus
  • Minimum of 2 years working in a community setting
  • Must be familiar or have experience working with uninsured/under-insured community members.
  • Must have knowledge of working with local and federal assistance applications.
  • Knowledge of Waukesha community resources a plus
  • Demonstrated stakeholder engagement skills
  • Must be fluent 100% in Spanish, orally and written
  • Strong written and oral skills for the use in the development of oral presentations, summaries and statistical reports
  • Ability to work independently as well as within an interdisciplinary team
  • Excellent time management, organizational, problem solving, interpersonal skills and a great team player
  • Demonstrated ability to work in a fast paced environment while keeping focus on task at hand.
