About the role

Key responsibilities & impact
  • Lead daily operations of the social services team, including policies, procedures, and care coordination with other clinical providers
  • Recruit, onboard, and mentor social workers, providing clinical supervision, observing practice, and supporting professional growth
  • Oversee assessment, planning, and evaluation of social services care
  • Ensure clinical documentation meets state, federal, and ACHC standards
  • Partner with the Executive Director on the annual department budget and resource planning
  • Build knowledge of community resources and connect patients and referral sources to appropriate support
  • Plan in-service training and continuing education
  • Contribute to quality improvement initiatives and identify ways to better serve patients
  • Perform other duties as required for patient services

Requirements

What you’ll need
  • Master of Social Work (MSW) from an accredited program
  • Active California license as an LCSW
  • At least 3 years of management experience, preferably in home care
  • Strong knowledge of healthcare regulations, ACHC standards, and business operations
  • Excellent communication skills and ability to build trust with patients, families, staff, and the community
  • Self-directed, organized, and flexible approach; initiative and ability to work well with others
