About the role

Key responsibilities & impact

Lead daily operations of the social services team, including policies, procedures, and care coordination with other clinical providers
Recruit, onboard, and mentor social workers, providing clinical supervision, observing practice, and supporting professional growth
Oversee assessment, planning, and evaluation of social services care
Ensure clinical documentation meets state, federal, and ACHC standards
Partner with the Executive Director on the annual department budget and resource planning
Build knowledge of community resources and connect patients and referral sources to appropriate support
Plan in-service training and continuing education
Contribute to quality improvement initiatives and identify ways to better serve patients
Perform other duties as required for patient services

Requirements

What you’ll need

Master of Social Work (MSW) from an accredited program
Active California license as an LCSW
At least 3 years of management experience, preferably in home care
Strong knowledge of healthcare regulations, ACHC standards, and business operations
Excellent communication skills and ability to build trust with patients, families, staff, and the community
Self-directed, organized, and flexible approach; initiative and ability to work well with others
