About the role

Key responsibilities & impact
  • Provide skilled nursing care to patients in their homes, primarily in Plymouth and surrounding areas
  • Utilize your critical thinking and assessment skills to develop and implement individualized care plans
  • Perform a range of essential nursing skills, including wound care, Foley catheter insertion and removal, PICC line and IV dressing changes, wound vacuum dressing changes, ostomy/ileostomy management, venipunctures, and Pleurex catheter drainage
  • Provide LNA supervision
  • Work independently while being a valued member of our collaborative team
  • Maintain accurate and timely documentation
  • Participate in a rotating weekend and on-call schedule to ensure continuous patient support
  • Travel to patient homes in various weather conditions

Requirements

What you’ll need
  • Current New Hampshire Registered Nurse (RN) license
  • Valid Driver's License and insured vehicle
  • Minimum of one year of professional nursing experience
  • Ability to work independently, strong self-motivation, excellent organizational skills, and the ability to travel
  • Demonstrated proficiency in wound care, Foley catheter insertion and removal, PICC line and IV dressing changes, wound vacuum dressing changes, ostomy/ileostomy management, venipunctures, and Pleurex catheter drainage
  • Associate's degree or higher in nursing
