About the role

  • Conduct comprehensive evaluations of speech, language, communication, and swallowing disorders.
  • Develop and implement individualized treatment plans to address patient goals.
  • Provide therapy for patients with neurological conditions, post-stroke complications, voice disorders, or other impairments.
  • Educate patients, caregivers, and families on therapy techniques, exercises, and communication strategies.
  • Collaborate with the interdisciplinary team to ensure holistic, patient-centered care.
  • Document clinical findings, progress notes, and treatment outcomes in accordance with PBH&HH standards and regulatory requirements.

Requirements

  • Master’s degree in Speech-Language Pathology from an accredited program.
  • Current NH licensure as a Speech Language Pathologist (or eligibility).
  • Previous experience in home health, hospice, or related healthcare setting strongly preferred.
  • Strong interpersonal, organizational, and communication skills.
  • Ability to work independently while being a collaborative team player.
