About the role
- Oversee comprehensive coordination of care and deliver skilled nursing visits to a dedicated caseload
- Conduct home visits every 60 days (or as needed) to reassess care needs for CNA and RN services
- Deliver skilled nursing care within RN scope of practice
- Complete orders and resumption of care assessments within 48 hours of client hospital discharge
- Educate clients and caregivers, providing guidance in Spanish on diagnosis, symptoms, and home care
- Coordinate with interdisciplinary teams and ensure alignment of care plans and physician orders
- Provide required CNA competencies and training
- Maintain professional communication with providers, clients, and families
- Travel frequently to client's homes within the Colorado Springs metro area
Requirements
- Active and unencumbered Colorado RN license with 1 year nursing experience required
- Current CPR/BLS card
- Valid driver’s license and reliable transportation
- Previous home health experience highly preferred
- Excellent communication and time-management skills
- Experience working in the disabled community highly preferred
- Must hold Colorado as primary state of residence (upload nursing license verifying this requirement)
- Comfortable with frequent travel to clients' homes in the Colorado Springs metro area
