About the role
- Conduct comprehensive evaluations and develop individualized treatment plans for home-based patients, coordinating with physicians and the care team as needed.
- Provide skilled, evidence-based occupational therapy interventions and adjust plans as patient needs evolve.
- Complete and score OASIS assessments accurately, collaborating with the Quality team to ensure compliance.
- Maintain knowledge of payer guidelines and Medicare Conditions of Participation.
- Participate in interdisciplinary case conferences and Quality Assurance & Performance Improvement (QAPI) initiatives.
- Supervise Occupational Therapy Assistants (OTAs) and provide oversight to CNAs when OT is the sole active discipline.
- Manage patient care throughout the episode, including admissions, 30-day functional assessments, and discharges.
- Document services and communications promptly and accurately using point-of-care tools.
- Coordinate daily schedules efficiently to maintain continuity of care and minimize travel time.
- Maintain current Montana OT licensure and complete all continuing education requirements.
- Adhere to ethical standards and agency policies while representing PIHC’s mission and values.
- Perform additional duties as assigned by the supervisor.
Requirements
- Bachelor’s degree or higher from an accredited Occupational Therapy program
- Current licensure to practice Occupational Therapy in the State of Montana
- Valid driver’s license and reliable transportation with appropriate auto insurance
- Two years of occupational therapy experience preferred (home health experience a plus)
