Physiotherapist
ParaMed Home Health CarePhysiotherapist responsible for assessment and rehabilitation of clients with various needs. Conducts assessments and collaborates with interdisciplinary teams in home or institutional settings.
About the role
Key responsibilities & impact- Conduct initial client assessments, identification of problems, and establishment of goals and treatment plans
- Develop client-centered plans with clients, inter-professional team and other stakeholders
- Engage clients using a collaborative approach, build rapport and trust, and achieve a mutual understanding and agreement related to the services to be provided
- Maintain professional client records related to client encounters and plans
- Work collaboratively with a multi-disciplinary team consisting of government agencies, healthcare workers, and other community resources
Requirements
What you’ll need- Bachelor's degree in Physiotherapy
- Registered and in good standing with the College of Physiotherapists of Ontario
- Willingness to travel to and from client’s residences within a designated area
- Valid driver license and access to a vehicle to travel to and from patient’s residences within a geographical area.
