About the role

Key responsibilities & impact
  • Assessing service needs and developing the New Jersey Individualized Service Plan (NJISP) for access to services.
  • Liaison with clients, families, programs, and service providers.
  • Establish achievable goals, outcomes, and service eligibility.
  • Development of a Person-Centered Planning Tool (PCPT).
  • Ongoing monthly monitoring of clients.
  • Coordinate and facilitate team meetings off-site at either Residential and/or Day Programs to identify service needs.

Requirements

What you’ll need
  • One year of proven experience working with adults with developmental disabilities.
  • Excellent written and verbal communication.
  • Reliable car and flexibility to attend meetings when needed during the day.
  • Must pass criminal background and CARI check.
  • A bachelor’s degree in psychology, sociology, social work, or related field.
