Bilingual Support Coordinator
NLC Support, LLCSupport Coordinator developing service plans for adults with developmental disabilities in New Jersey. Managing client needs, monitoring services, and facilitating team meetings across various programs.
About the role
Key responsibilities & impact- Assessing service needs and developing the New Jersey Individualized Service Plan (NJISP) for access to services
- Liaison with clients, families, programs, and service providers
- Establish achievable goals, outcomes, and service eligibility
- Development of a Person-Centered Planning Tool (PCPT)
- Ongoing monthly monitoring of clients
- Coordinate and facilitate team meetings off-site at Residential and/or Day Programs
Requirements
What you’ll need- One year of proven experience working with adults with developmental disabilities
- Excellent written and verbal communication
- Reliable car
- Flexibility to attend meetings when needed during the day
- Bachelor’s degree in psychology, sociology, social work, or related field
- Must pass criminal background and CARI check
- Bilingual Spanish required
