About the role

Key responsibilities & impact
  • Assessing service needs and developing the New Jersey Individualized Service Plan (NJISP) for access to services
  • Liaison with clients, families, programs, and service providers
  • Establish achievable goals, outcomes, and service eligibility
  • Development of a Person-Centered Planning Tool (PCPT)
  • Ongoing monthly monitoring of clients
  • Coordinate and facilitate team meetings off-site at Residential and/or Day Programs

Requirements

What you’ll need
  • One year of proven experience working with adults with developmental disabilities
  • Excellent written and verbal communication
  • Reliable car
  • Flexibility to attend meetings when needed during the day
  • Bachelor’s degree in psychology, sociology, social work, or related field
  • Must pass criminal background and CARI check
  • Bilingual Spanish required
