About the role
Key responsibilities & impact- Assessing service needs and developing the New Jersey Individualized Service Plan (NJISP) for access to services
- Liaison with clients, families, programs, and service providers
- Establish achievable goals, outcomes, and service eligibility
- Development of a Person-Centered Planning Tool (PCPT)
- Ongoing monthly monitoring of clients
- Coordinate and facilitate team meetings off-site at Residential and/or Day Programs
Requirements
What you’ll need- One year of proven experience working with adults with developmental disabilities
- Excellent written and verbal communication
- Reliable car
- Flexibility to attend meetings when needed during the day
- Bachelor’s degree in psychology, sociology, social work, or related field
- Must pass criminal background and CARI check
- Bilingual Spanish required
