About the role

Key responsibilities & impact

Conduct comprehensive health and psychosocial evaluations and monitor each patient's condition
Coordinate total patient care, including health promotion, prevention, service coordination, and teaching
Determine the need for and frequency of CHHA services based on patient and home assessments
Develop and implement individualized nursing care plans and evaluate their effectiveness
Complete admission, transfer, re-certification, resumption-of-care, and discharge documentation
Submit clinical and progress notes weekly and a daily tally of patient visits
Consult with attending physicians on care plan changes and involve the supervisor as needed
Present records to the Clinical Record Review Committee and participate in case conferences
Explain nursing and firm services to patients and families during care planning
Participate in firm accreditation activities and surveys

Requirements

What you’ll need

Active NJ Registered Nurse license in good standing
Mid-level clinical experience
Home care or I/DD experience preferred
Comfort working independently in a remote-first workflow
Reliable transportation for in-person visits
Strong documentation habits
Clear communication with patients, families, and physicians
