About the role
Key responsibilities & impact- Conduct comprehensive health and psychosocial evaluations and monitor each patient's condition
- Coordinate total patient care, including health promotion, prevention, service coordination, and teaching
- Determine the need for and frequency of CHHA services based on patient and home assessments
- Develop and implement individualized nursing care plans and evaluate their effectiveness
- Complete admission, transfer, re-certification, resumption-of-care, and discharge documentation
- Submit clinical and progress notes weekly and a daily tally of patient visits
- Consult with attending physicians on care plan changes and involve the supervisor as needed
- Present records to the Clinical Record Review Committee and participate in case conferences
- Explain nursing and firm services to patients and families during care planning
- Participate in firm accreditation activities and surveys
Requirements
What you’ll need- Active NJ Registered Nurse license in good standing
- Mid-level clinical experience
- Home care or I/DD experience preferred
- Comfort working independently in a remote-first workflow
- Reliable transportation for in-person visits
- Strong documentation habits
- Clear communication with patients, families, and physicians
