About the role
Key responsibilities & impact- Initiate, develop, and implement basic nursing plans of care and treatments, evaluating patient progress
- Provide direct patient care, perform treatments, administer medications, and educate patients and caregivers
- Modify treatment plans based on changing patient status and physician orders
- Assess and re-evaluate patient needs and physical status at each visit
- Obtain and clarify physician orders and promptly inform physicians of significant patient-condition changes
- Teach patients and families to independently manage prescribed therapy and care needs
- Communicate with the clinical team, medical providers, patients, and caregivers to facilitate continuity of care
- Prepare and submit real-time clinical documentation during visits
- Coordinate discharge planning and prepare discharge summaries under close supervision
- Comply with agency policies, professional standards, regulations, guidelines, and accreditation standards
- Use computers and automation technologies to validate orders, communicate care provided, and follow assigned visit schedules
- Respond to patient requests and initiate emergency procedures when necessary
- Safeguard confidential patient and company information, including HIPAA compliance
- Instruct patients about Naven Health's scope of services and plans of care
- Provide oversight and coordination of paraprofessionals in the home setting
- Participate in multidisciplinary conferences and precept, train, and mentor nurses as assigned
- Participate in nursing on-call responsibilities as applicable
- Perform other related duties as directed by the supervisor
Requirements
What you’ll need- Active and unrestricted RN license in the state of practice
- Minimum of 0–2 years of infusion nursing experience required
- Current CPR certification required; an in-person class is required per company policy
- Demonstrated competency in patient care standards and infusion skill sets applicable to agency programs
- Advanced certification and training as applicable
- Required licensure to operate a motor vehicle in the state of practice
- Access to a vehicle for business travel with proof of liability insurance
- Basic knowledge of computer operating systems and software applications
- Ability to use nursing technology tools to communicate and document care
- Ability to lift up to 50 pounds, with a maximum lifting capacity of 75 pounds
- Ability to perform required physical activities, including walking, standing, crouching, kneeling, reaching, grasping, pushing, pulling, lifting, and carrying
- Fine motor skills and visual acuity, including near, distance, color, peripheral, and depth perception
- Ability to comply with safety requirements, including closed-toe shoes and protective equipment
- Willingness to travel 100% of the time for business purposes
- Bachelor of Science in Nursing from an accredited school of nursing preferred
- One year of previous RN experience in home health or acute care preferred
