About the role

  • Using a 'whole person' approach, provide case management for veterans. Refer and assist clients to develop saleable skills, work habits, and coping skills. Work with homeless and disadvantaged veterans, male and female. Network with local service organizations providing shelter, medical, mental health, legal, transportation, employment and other services. Facilitate client wellness and autonomy through advocacy, assessment, planning, education, resource management, and service facilitation. Screen clients for suitability and eligibility for programs; make referrals and act as advocate. Apply outreach techniques and develop outreach program to engage homeless veterans on the street and in encampments. Travel to referral agencies and clients’ homes for outreach, intakes and follow-ups. Ensure residents comply with program rules and monitor funding agency performance goals. Maintain complete participant documentation, conduct preliminary assessments, develop Individualized Service Plans (ISPs), and update goals. Provide expertise to recognize clinical diagnosis and make referrals; does not provide therapy. Provide strength-based, housing-focused plans and coordinate integrated services as member of Client Review Team. Maintain resource file of social service contacts and landlords; provide employment counseling and job readiness workshops. Coordinate landlord recruitment and retention; perform additional duties as assigned by supervisor.

Requirements

  • Bachelor’s degree with Two years of college level coursework in related field (psychology, sociology) preferred. Two years’ experience in case management within a social service or employment counseling position required. Experience working with AOD/Mental Health clients preferred. Experience performing outreach and making referrals to services. Broad working knowledge of services within the local area. Demonstrable knowledge and advocacy of veteran and children’s issues. Knowledge of native culture and language (if applicable), as well as a deep familiarity with the veteran issues relevant to this area. Excellent organizational and time-management skills. Proficient in Word, Excel and Outlook programs, and familiar with entering data in database programs. Must have clean DMV record and reliable transportation. Veteran status preferred, but not required.
