About the role

Key responsibilities & impact
  • Meet with trainees to complete individualized assessments that identify career goals, strengths, barriers, support needs, and readiness for training and employment.
  • Develop and maintain personalized career plans that outline short and long-term goals, action steps, timelines, and follow-up needs.
  • Support trainees in understanding career pathways from entry-level direct care roles, including PCA and HHA, to advancement opportunities such as CNA and related health care positions.
  • Provide ongoing coaching, encouragement, and accountability to promote attendance, engagement, training completion, and successful transition to employment.
  • Provide education and coaching on essential life skills, including communication, professionalism, time management, conflict resolution, financial literacy, self-advocacy, and workplace readiness.
  • Assist participants with identifying and addressing barriers related to transportation, childcare, tutoring, technology access, emotional support, housing instability, food insecurity, or other needs that may affect training or employment success.
  • Connect trainees with appropriate community resources, public benefits, support services, and partner organizations across the Central New York region.
  • Maintain accurate, timely, and confidential documentation of participant assessments, individualized plans, referrals, follow-up contacts, progress, outcomes, and barriers addressed.
  • Collaborate with program leadership to support Direct Caregiver Flexibility Grant deliverables, data collection, quality improvement activities, and required reports.
