About the role

Key responsibilities & impact

Primary Care Services: Conduct physical exams, diagnose and treat acute and chronic illnesses, and provide routine primary care to improve patient health outcomes.
Patient Assessments: Take medical histories, perform physical examinations, and order and interpret diagnostic tests.
Chronic Disease Management: Develop and manage treatment plans for chronic conditions such as diabetes, hypertension, and asthma.
Documentation: Maintain accurate and timely patient records using electronic health record (EHR) systems.
Administrative Duties: Including but not limited to assisting with opening/closing the clinic, inventory, patient follow-up, verifying insurance, collecting payment, scheduling appointments, and maintaining a clean environment.
Preventive Care: Administer vaccinations, conduct health screenings, and educate patients on disease prevention and healthy lifestyles.
Collaboration: Work with other healthcare providers, nurses, and medical assistants to deliver comprehensive care.
Community Outreach: Participate in outreach activities to raise awareness about the mobile clinic’s services and engage with the community.
Actively participating in leadership-level planning for the best growth opportunities.

Requirements

Education: Master’s or DNP Family Nurse Practitioner (FNP) with completion of an accredited FNP program, with current Family Nurse Practitioner (ANCC or AANPCB) Board Certification or equivalent certification.
Currently licensed or eligible for California APRN licensure as a Family Nurse Practitioner
Experience: Clinical experience in family and primary care services preferred
Experience with EHR preferred
Performance: Able and willing to provide world-class service to all stakeholders.
