Bilingual Advanced Practice Nurse, Memory Support
Millennium Physician GroupBilingual Advanced Practice Registered Nurse providing telehealth evaluations and dementia care. Responsible for patient assessments and caregiver education via virtual platforms.
About the role
Key responsibilities & impact- Conduct comprehensive telehealth evaluations for patients with memory concerns, including detailed clinical histories and remote physical/neurological observations.
- Administer and interpret validated remote assessment tools (e.g., Mini-Cog, MoCA, SLUMS) to determine the stage and type of cognitive impairment.
- Diagnose and manage neuropsychiatric complications such as agitation, anxiety, depression, and sleep disturbances using both pharmacological and non-pharmacological interventions.
- Educate families on disease progression, safety planning (wandering, driving), and behavioral de-escalation techniques.
- Maintain electronic health records (EHR) in compliance with Florida state regulations and HIPAA standards for telehealth.
Requirements
What you’ll need- Must hold a current, unencumbered Florida APRN license with prescriptive authority.
- Master’s or Doctoral degree in Nursing (MSN or DNP).
- Minimum of 2–3 years of experience specifically in geriatrics, neurology, or a dedicated memory care setting preferred.
- Experience conducting video-based clinical visits and comfort troubleshooting basic technology issues with elderly patients and their families.
- Working knowledge of the CMS Cognitive Assessment & Care Plan requirements and the nuances of dementia-related behavioral health.
