About the role
- Create and track patients' care plans
- Accompany patients in their healthcare journey
- Regularly engage with patients and track their adherence to their care plans.
- Closely monitor patients' device readings and dig into any out-of-bound values.
- Maintain meticulous records of patient interactions and program details.
- Stay up-to-date with the latest regulations and compliance standards related to CCM/RPM services.
- Ensure all patient enrollments adhere to legal and ethical standards.
- Monitor and track care performance, striving to meet or exceed goals.
Requirements
- National Certification in Medical Assisting
- Residence in the US
- Strong communication skills, both written and verbal.
- Empathy and the ability to connect with patients and their families.
- Basic knowledge of chronic health conditions and their management.
- Comfortable using technology for documentation and communication.
- Attention to detail and strong organizational skills.
- Ability to work independently and as part of a team.
