About the role

Key responsibilities & impact

Create and track patients' care plans
Accompany patients in their healthcare journey
Regularly engage with patients and track their adherence to their care plans.
Closely monitor patients' device readings and dig into any out-of-bound values.
Maintain meticulous records of patient interactions and program details.
Stay up-to-date with the latest regulations and compliance standards related to CCM/RPM services.
Ensure all patient enrollments adhere to legal and ethical standards.
Monitor and track care performance, striving to meet or exceed goals.

Requirements

What you’ll need

National Certification in Medical Assisting
Residence in the US
Strong communication skills, both written and verbal.
Empathy and the ability to connect with patients and their families.
Basic knowledge of chronic health conditions and their management.
Comfortable using technology for documentation and communication.
Attention to detail and strong organizational skills.
Ability to work independently and as part of a team.
