About the role

Key responsibilities & impact

Provide high quality, evidence based comprehensive virtual primary care
Conduct a thorough history and assessment of symptoms virtually
Perform urgent care and medical triage visits as needed
Support the Transitions of Care nurse team by addressing urgent patient care needs
Offer patient education, counseling, and follow-up care
Assist PCPs in electronic medical record (EHR) inbox management
Complete documentation in the electronic health record (EHR) within MCC timeliness standards

Requirements

What you’ll need

Advanced Practice Provider (NP/PA) from an accredited medical training program
Current California medical license and DEA certificate required
Minimum of 3 years of experience providing primary care in a family or community health setting
Experience with EHR systems (EPIC preferred)
Bilingual (Spanish/English) desired
Active BLS Certification
Must maintain Continued Medical Education (CME) as required for certification renewals.
