About the role

Key responsibilities & impact
  • Provide comprehensive assessment of social, emotional, and practical needs to patients and families in outpatient PC clinic, private residences, and care facilities.
  • Introduce and educate regarding Palliative Care and Hospice services.
  • Conduct independent psychosocial assessment/evaluations and provide 1:1 psychotherapy.
  • Participate in professional team reviews of patient care and cases.
  • Provide casework and group work for patients and families.
  • Identify communication resources and assist patients and families in connection with resources and advance care planning.
  • Responsible for continuing professional development to enhance practice.

Requirements

What you’ll need
  • MSW from a CSWE accredited School of Social Work.
  • Licensed Clinical Social Work license required.
  • Completion of American Cancer Society Leadership in Oncology Navigation / PIN training and credentialing within one year of hire.
  • At least two years of professional social work experience, preferably in a health care setting.
