Work with people we support and their families and friends to assess needs
Determine desired outcomes and coordinate services
Refer to community resources
Create and implement Individual Service Plans (ISP) for youth ages 13-22 in Licking County

Requirements

What you’ll need

Minimum of an Associates Degree
Bachelors Degree preferred
Well organized and detail oriented
Ability to meet deadlines
Passion and enthusiasm to make a difference in the lives of young people with developmental disabilities
