About the role

Key responsibilities & impact
  • Conduct initial and supervisory visits to clients in their homes to assess care quality, train attendants, and ensure service plan compliance.
  • Complete and submit documentation accurately and in a timely manner following each field visit.
  • Support office operations, including follow-up calls, scheduling, paperwork, and coordination, when not conducting visits.
  • Respond to urgent care issues or service interruptions in the field.
  • Communicate professionally with clients, caregivers, and staff to support quality outcomes and satisfaction.
  • Participate in training, audits, and other projects as assigned by the Office Manager.

Requirements

What you’ll need
  • May have completed two (2) years of full-time study at an accredited college or university; OR may substitute one year of full-time employment in a supervisory capacity in a health care setting for each required year of college.
  • Reliable transportation and ability to travel locally for home visits.
  • Excellent communication and organizational skills.
  • Ability to adapt between field and office roles as needed.
  • Proficient in Microsoft Office and comfortable using electronic documentation systems.
  • Bilingual (English/Spanish)
