About the role
- Assessing the individual needs of beneficiaries using the BEI_NRW needs-assessment instrument
- Implementing service planning
- Managing the delivery of services
- Conducting evaluations
- Coordinating collaboration with the payer’s case management
- Ensuring timely, professionally qualified, and needs-oriented participation planning
- Serving as the contact person for beneficiaries and for the payer regarding individual participation management
Requirements
- At least 2 years of relevant professional experience in residential or outpatient supported living within integration assistance
- Initial experience completing the BEI_NRW
- Knowledge of the ICF and the BTHG
- Strong communication skills
- Ability to multitask
- Independent, reliable, and proactive working style
- Proficiency with common M365 applications
