About the role
Key responsibilities & impact- Evaluate, assess progress and provide plan of care and exercise prescription for patient caseload
- Consistently ensure a world-class level of clinical care and patient experience
- Document all treatments in our EMR
- Communicate via chat and phone with patients and/or providers as needed
Requirements
What you’ll need- Current Licensed Physical Therapist in New York State
- Completed state regulated CEU requirement in NYS
- BLS certification
- Commit to a minimum of 4 visits/week
- Demonstrated excellent written & verbal communication skills
- Excited to build and deliver a new model for achieving better clinical outcomes
- Creative and flexible, but always puts the patient first
- Excellent at forging successful and respectful relationships with the entire team
