About the role

Key responsibilities & impact
  • Assess patients’ knowledge of late-stage CKD and treatments, and provide education to support informed decisions during transition to RRT
  • Provide support, guidance, and care coordination for patients seeking conservative or palliative care
  • Liaise with appropriate staff to ensure patients and family members receive comprehensive information on dialysis modalities, transplantation, conservative care, and hemodialysis access types
  • Organize late-stage CKD patient populations regarding education, modality selection, permanent access placement and maintenance, and transition to RRT
  • Use motivational interviewing techniques to encourage members to engage in their own care
  • Participate in interpreting summary clinical data and using it to improve late-stage CKD care processes

Requirements

What you’ll need
  • Minimum of 6 months previous experience as a Renal Care Coordinator with Interwell Health
  • 1–2 years of prior Renal Care Coordinator experience highly preferred
  • Combination of renal transplant, dialysis, or CKD patient care required
  • Strong organizational and communication skills
  • Must be available to work hours of the time zone of supported practices
