About the role

Key responsibilities & impact

Meet with assigned clients 3 times per month
Perform rehabilitation activities at the office, in clients’ homes, in the community, or remotely if age appropriate
Complete and submit thorough, accurate, and timely documentation including in encounter notes, monthly summaries, Individual Rehabilitation Plans, discharge/transition recommendations and summaries; in accordance with agency policy and COMAR regulations
Maintain consistent contact with clients and work cooperatively with other members of the client’s treatment team
Uphold with high regard, the confidentiality of clients and follow all HIPPA guidelines.

Requirements

What you’ll need

Associates or Bachelor’s degree in a mental health or human services related field
OR 30 hours or equivalent of college credit towards a Bachelor’s degree in a health related field
OR a High school diploma with 1 year of work experience in a supervised mental health setting
Must possess or obtain certification in First Aid and CPR
Must pass state and Child Protective background checks
Active, unrestricted Maryland driver’s license, reliable personal vehicle and active car insurance
Must reside within a sustainable driving distance for monthly on-site visits
Must be able to provide on-site client services once per month as we provide a blended service.
