About the role

Key responsibilities & impact
  • Build and refine scalable care management processes, operating standards, and best practices for a new agency
  • Complete client assessment packets and provide signed agreements to clients or representatives
  • Develop and implement written care plans with clients or designated representatives
  • Collect fees for initial services
  • Introduce caregivers during the first scheduled shift and review care plans and service expectations
  • Supervise agency personnel delivering client care and ensure compliance with care plans, infection control, emergency procedures, licensing scope, and safety requirements
  • Communicate regularly with clients and representatives to follow up on service delivery and satisfaction
  • Perform scheduled and unscheduled in-home client support visits
  • Conduct reassessments based on agency frequency and changes in client condition or events
  • Coordinate services involving other care agencies, hospice agencies, facilities, or community services
  • Serve as a resource for clients and families through education, empowerment, recommendations, and referrals
  • Investigate client-care and client or employee well-being issues
  • Document supervision and maintain current client records in agency systems
  • Provide staff training and education in clients’ homes, by telephone, and in the agency office
  • Report client and employee information to agency staff for service delivery, caregiver matching, scheduling, and management
  • Assist with developing and revising client care policies and procedures
  • Participate in caregiver personnel evaluations

Requirements

What you’ll need
  • Two years of experience in home care, home health, or a related field
  • Reliable means of transportation with current insurance and a valid driver’s license
  • Excellent written and verbal communication skills
  • Ability to interact effectively with clients, families, care providers, and other professionals
  • Impeccable follow-up and attention to detail
  • Ability to exercise initiative, solve problems, and apply sound judgment
  • Ability to supervise, assess, and evaluate care providers
  • Ability to advocate on behalf of clients and families
  • Passion for helping others live with dignity
  • Positive and professional business image
  • Must pass a criminal background check
  • Credentials may affect compensation
