About the role
Key responsibilities & impact- Assess & Plan Complete in-home RN assessments; craft individualized Care Plans with client/family approval.
- Monitor & Document Review daily care logs, track condition changes, and interventions in our HIPAA-secure EMR; escalate concerns promptly.
- Collaborate & Advocate Coordinate with physicians, therapists, and schedulers to keep care seamless and client-centered.
Requirements
What you’ll need- Active Registered Nurse (RN) license in Georgia
- Bachelor’s in Nursing or related human-services field – OR – 2+ years’ geriatric/home-care experience
- Solid clinical assessment skills and a passion for problem-solving
- CPR/First Aid certification and a valid driver’s license
- Warm, professional communication that builds trust with families and field staff
