About the role

Key responsibilities & impact
  • Assess & Plan Complete in-home RN assessments; craft individualized Care Plans with client/family approval.
  • Monitor & Document Review daily care logs, track condition changes, and interventions in our HIPAA-secure EMR; escalate concerns promptly.
  • Collaborate & Advocate Coordinate with physicians, therapists, and schedulers to keep care seamless and client-centered.

Requirements

What you’ll need
  • Active Registered Nurse (RN) license in Georgia
  • Bachelor’s in Nursing or related human-services field – OR – 2+ years’ geriatric/home-care experience
  • Solid clinical assessment skills and a passion for problem-solving
  • CPR/First Aid certification and a valid driver’s license
  • Warm, professional communication that builds trust with families and field staff
