About the role
Key responsibilities & impact- Serve as a core healthcare team member supporting care managers in managing the overall health of enrolled populations
- Improve recognition, treatment, and management of psychosocial, behavioral, and medical conditions
- Provide clinical mental health consultation services within the scope of state professional licensure
- Develop clinical pathways and best-practice programs for targeted patient groups
- Participate in staff meetings, trainings, daily huddles, and the system-wide BHC learning collaborative
- Educate and support multidisciplinary team members on behavioral health and substance abuse topics
- Provide population-based care to high-risk and high-need patients
- Document in the shared medical record
- Engage patients by telephone and in person
- Provide same-day assessment, intervention, and referrals to higher levels of mental health and addiction care
- Deliver brief therapeutic counseling interventions in the ambulatory care environment
- Use solution-focused approaches, CBT, Motivational Interviewing, and other short-term counseling interventions
- Coordinate with patient family members and support systems with patient permission
- Provide brief follow-up and relapse-prevention education
- Promote skill building for psychological and physical health, including groups when appropriate
- Assist patients in crisis and triage or refer them to specialty mental health and substance abuse services
- Advocate for patients, re-engage patients who are not participating in care, and encourage active treatment participation
- Connect patients with community resources related to social determinants of health
Requirements
What you’ll need- Master’s degree in Mental Health Field
- 2 years experience in use of evidence-based counseling techniques, screening, diagnosis, and treatment of common mental health and substance abuse disorders
- Knowledge of the relationship between medical and psychological aspects of health and disease
- Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage and Family Therapist (LMFT)
- CPR – American Heart Association
- Act 34 Criminal Background Clearance Certificate
- Act 33 Child Abuse Clearance Certificate
- Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
- Compliance with HIPAA, data security guidelines, the Code of Business Conduct, applicable laws, regulations, company policies, and training requirements
