Key responsibilities & impact

Provide clinical treatment services to targeted populations in an outpatient treatment program for patients with mental health diagnoses
Develop and facilitate assessments, therapeutic interventions, treatment plans, and encounter forms for assigned patients and/or providers
Ensure continuity of services through clinical decision-making and compliant documentation, treatment, and case management
Discuss additional service needs with the clinical team and determine appropriate recommendations
Refer patients to other levels of care, community-based recovery self-help programs, and social or human services
Act as an internal and external liaison with professional services to meet patient care requirements
Participate in assigned internal and external meetings
Participate in program quality assurance and performance improvement activities

Requirements

What you’ll need

Minimum Master’s degree in Psychology, Social Work, Counseling, Education (School Counseling), or related field
CPR certification from the American Heart Association
Act 34 Criminal Background Clearance Certificate
Act 33 Child Abuse Clearance Certificate
Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
Compliance with HIPAA, data security guidelines, company policies, applicable laws, and the Code of Business Conduct
