About the role
Key responsibilities & impact- Responsible for leading professional development for Care Management services across assigned areas of the health system
- Designs and evaluates onboarding, annual education, continuing education, and competency validation
- Collaborates with Care Management leadership and interdisciplinary teams
- Plans, develops, implements, and evaluates Care Management-focused educational and professional development programs
- Serves as an educational consultant to leadership and staff
Requirements
What you’ll need- Master’s degree in Social Work, Nursing, Education or relevant experience
- 2 years of experience in Hospital-based Care Management clinical
- 2 years of experience in Education and training
- Current State of PA RN or PA SW licensure or current multi-state licensure
- Certification in Case Management (CCM®, ACM®, or equivalent) within 3 years of hire
- CPR certification
- Act 34 Criminal Background Clearance Certificate
- Act 33 Child Abuse Clearance Certificate
- Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
