About the role
Key responsibilities & impact- Serve as the consistent point of contact for referring physicians, patients, caregivers, families, and the community
- Facilitate patient flow by coordinating appointments and access to clinical and supportive care services
- Coordinate all aspects of care for adult patients with specialized disease states in collaboration with the multidisciplinary team
- Provide education, screening, support, referrals, care coordination, and other necessary assistance
- Oversee community outreach, high-risk populations, and disease-specific sites
- Educate patients and coordinate care regarding diagnoses, treatment options, treatment courses, clinical trials, and available resources
- Coordinate safe handoffs and transitions of care between facilities and inpatient/outpatient settings
- Work with High-Risk Care Teams to reduce readmissions
- Assist Registry staff with data collection, patient outcomes, and care delivery model updates as needed
- Coordinate multidisciplinary appointments and accompany patients as needed
- Address medication adherence issues
- Identify at-risk patients and facilitate appropriate screening processes
- Trend navigation-program data and outcomes and identify care gaps
- Develop or attend clinical care conferences and report active patients to the multidisciplinary team
- Identify bereavement needs and develop family plans of care as needed
- Perform other duties as assigned or required
Requirements
What you’ll need- Current State of PA RN licensure OR current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC)
- 1–3 years of nursing experience in the specialty
- CPR certification from the American Heart Association
- Act 34 Criminal Background Clearance Certificate
- Act 33 Child Abuse Clearance Certificate
- Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
- Valid PA driver's license may be required
- Older Adults Protective Services Act (OAPSA) clearance may be required depending on location
- Bachelor's Degree in Nursing preferred
- Certification in area of expertise preferred
- Compliance with HIPAA, data security guidelines, company policies, code of business conduct, applicable laws, regulations, and training requirements
