About the role
- Develop and maintain positive relationships with clients, their families, and other stakeholders to understand their unique needs and preferences
- Conduct comprehensive assessments to evaluate clients' physical, emotional, and social needs
- Collaborate with clients to develop individualised support plans and coordinate the delivery of various disability services
- Advocate on behalf of clients to ensure they receive the appropriate level of funding and access to relevant services
- Monitor and review client progress, adjusting support plans as needed
- Maintain detailed client records and comply with all relevant policies and procedures
- Contribute to the continuous improvement of our disability support services
Requirements
- Relevant tertiary qualification in social work, disability studies, or a related field
- Minimum 2 years' experience working in the disability sector, preferably as a case manager or support coordinator
- Excellent communication and interpersonal skills, with the ability to build trust and rapport with clients
- Strong problem-solving and decision-making skills, with a focus on person-centred, outcomes-based support
- Familiarity with the National Disability Insurance Scheme (NDIS) and other relevant government programs and legislation
- Flexible and adaptable, with the ability to work independently and as part of a team
- Empathetic, compassionate, and committed to making a positive difference in the lives of people with disabilities
