About the role

Key responsibilities & impact
  • The Nurse (RN or LPN) is responsible for nursing care at patient appointments
  • Obtaining vital signs and patient history
  • Providing individualized education to patients related to acute and chronic diagnosis
  • Triaging patient calls and schedules visits as appropriate
  • Administering oral and injectable medication and treatments within the scope of practice and with orders from providers
  • May distribute sample medication per provider orders
  • May perform venipuncture blood draws and urine collection for lab work as ordered by the provider
  • Maintaining patient data and health information in the electronic medical record according to documentation guidelines
  • Generating patient-related reports, as needed
  • Participating in performance improvement programs and disease management programs
  • Assisting in the coordination of well-child and immunization programs
  • Assisting in ordering and stocking clinical supplies
  • Completing transitional care management follow-up for patients with hospital admissions
  • Monitoring assigned AthenaOne Buckets for patient follow-up according to outline protocol, including patient portal messages.
  • Monthly participation on the Life Risk Safety Committee
  • Routine safety and risk assessment to support Just Culture.
  • Supporting care coordination efforts as requested by the primary care provider

Requirements

What you’ll need
  • Graduate of a Nursing Education Program
  • Current unrestricted Registered Nursing (RN) or Licensed Practical Nurse (LPN) License for the state of NH
  • BLS certification required
  • One year of nursing experience preferred.
