About the role
Key responsibilities & impact- The Nurse (RN or LPN) is responsible for nursing care at patient appointments
- Obtaining vital signs and patient history
- Providing individualized education to patients related to acute and chronic diagnosis
- Triaging patient calls and schedules visits as appropriate
- Administering oral and injectable medication and treatments within the scope of practice and with orders from providers
- May distribute sample medication per provider orders
- May perform venipuncture blood draws and urine collection for lab work as ordered by the provider
- Maintaining patient data and health information in the electronic medical record according to documentation guidelines
- Generating patient-related reports, as needed
- Participating in performance improvement programs and disease management programs
- Assisting in the coordination of well-child and immunization programs
- Assisting in ordering and stocking clinical supplies
- Completing transitional care management follow-up for patients with hospital admissions
- Monitoring assigned AthenaOne Buckets for patient follow-up according to outline protocol, including patient portal messages.
- Monthly participation on the Life Risk Safety Committee
- Routine safety and risk assessment to support Just Culture.
- Supporting care coordination efforts as requested by the primary care provider
Requirements
What you’ll need- Graduate of a Nursing Education Program
- Current unrestricted Registered Nursing (RN) or Licensed Practical Nurse (LPN) License for the state of NH
- BLS certification required
- One year of nursing experience preferred.
