About the role
Key responsibilities & impact- Conduct nursing assessments in the home setting, identifying the clinical, functional, emotional, and social needs of oncology patients.
- Perform home visits and nursing telecare, ensuring longitudinal follow-up and continuity of care.
- Work on the identification, assessment, and eligibility of patients for Laços Saúde assistance programs according to established criteria.
- Develop, implement, and monitor individualized care plans aligned with the patient’s needs and the multidisciplinary team’s directives.
- Monitor clinical progression, treatment-related symptoms, and potential complications, initiating early interventions when necessary.
- Provide guidance to patients, family members, and caregivers on treatment, symptom management, safe medication use, complication prevention, and self-care.
- Carry out health promotion activities, prevention of adverse events, and maintenance of functionality throughout treatment.
- Coordinate care with the multidisciplinary team, arranging referrals and following up on the execution of the care plan.
- Document assessments, progress notes, interventions, and care indicators in the electronic medical record and the platforms used by the organization.
- Participate in clinical meetings, case discussions, and the collaborative development of therapeutic plans.
- Prepare care reports and make reports according to the organization’s processes and governance model.
- Attend trainings, continuing education, and professional development programs promoted by Laços Saúde.
- Act in accordance with care protocols, ethical principles, and institutional values, ensuring safe, person-centered, and evidence-based care.
Requirements
What you’ll need- Bachelor’s degree in Nursing.
- Active registration with the Regional Nursing Council (COREN).
- Completed postgraduate specialization in Oncology.
- Clinical experience as a nurse.
- Availability to perform home visits.
- Experience in longitudinal follow-up of oncology patients.
- Experience in home care for oncology patients or in palliative care.
- Knowledge of patient navigation, care management, and coordination of the oncology care pathway.
