About the role

Key responsibilities & impact

Act in accordance with the organization’s values, maintaining a results-oriented approach to care and respecting each patient’s individuality to provide personalized assistance
Document essential and relevant information in the Patient’s Medical Record, keeping it up to date
Represent the organization, upholding its principles and delivering care based on scientific evidence and best practices
Provide comprehensive patient care with empathy and compassion, building therapeutic relationships and addressing patients’ biopsychosocial needs
Ensure continuity of medical care by implementing the established Care Plan
Properly allocate the appointment schedule, meeting the contracted working hours and number of consultations

Requirements

What you’ll need

Medical degree with Residency or Postgraduate training in Geriatrics and/or Family and Community Medicine
Active registration with the Regional Medical Council (CRM)
Prior experience in home-based care or community health is a plus
Passion for promoting self-care and patient autonomy, with a person-centered approach
Strong communication skills and empathy, essential for building rapport with patients and families
Ability to work collaboratively within multidisciplinary teams, integrating actions that promote patient health in their family environment
Proactive in seeking solutions that enhance patients’ quality of life and functional capacity
