About the role

Key responsibilities & impact
  • Provide specialist geriatric and complex care to patients across the Grampians region in hospital and community settings
  • Provide direct and remote telehealth assessment, complex management planning, prescribing and collaborative team decision-making
  • Lead and support the multidisciplinary team to achieve documented patient and team goals
  • Coordinate care with nursing, allied health, diagnostic and community services
  • Supervise, teach and mentor junior medical staff allocated to Hospital Without Walls and Geriatric Medicine
  • Work with clinicians across the region to develop and deliver community-based services
  • Contribute to education, collaborative strategic planning, policy and procedure development, quality improvement and patient consultation
  • Ensure complete, high-quality patient-care documentation and evidence-based practice

Requirements

What you’ll need
  • MBBS or equivalent degree
  • Current medical registration in Australia
  • Fellowship of the Royal Australasian College of Physicians (FRACP) or the Royal Australian College of General Practitioners (FRACGP), or an equivalent qualification recognised by the Royal Colleges
  • Demonstrated experience and skills in the medical assessment and clinical management of complex older patients
  • Detailed knowledge of the investigation and management of acute and chronic medical disorders
  • Understanding of the impact of illness, disability and ageing on patients and their families
  • Ability to lead a multidisciplinary team and operate effectively at a senior clinical level
  • Excellent written and verbal communication skills
  • Ability to work effectively independently and as part of a team
  • Commitment to quality improvement, ongoing professional development and evidence-based practice
  • Satisfactory police records check
  • Immunisation clearance
  • Working with Children check and/or NDIS Worker Screening Check
  • Willingness and ability to wear Personal Protective Equipment (PPE) when required
