About the role

Key responsibilities & impact

Provide companionship and, if authorized, transportation according to the care plan
Assist clients with activities of daily living, including bathing, meals, light housekeeping, and safe ambulation/transfers
Report changes in clients’ conditions to the supervisor
Document care provided and maintain accurate agency records
Collaborate with the client’s care team and ensure effective communication
Promote client safety through infection-control practices and reporting unsafe conditions
Attend in-service programs to meet compliance requirements and notify supervisors of educational needs
Provide personal care services under the direction of the Registered Nurse or Therapist
Support children and adults who are aging in place, medically complex, or living with chronic conditions

Requirements

What you’ll need

Successful completion of a formal certification training program and/or a written skills test and competency evaluation
Be at least eighteen (18) years of age
Demonstrated ability to read, write, and follow a written care plan
Good verbal, written, and interpersonal skills
Licensed driver with an automobile insured according to state and/or Agency requirements and in good repair
Current CPR certification
Current TB test results
Current CNA license
