About the role
Key responsibilities & impact- Provide companionship and, if authorized, transportation according to the care plan
- Assist clients with activities of daily living, including bathing, meals, light housekeeping, and safe ambulation/transfers
- Report changes in clients’ conditions to the supervisor
- Document care provided and maintain accurate agency records
- Collaborate with the client’s care team and ensure effective communication
- Promote client safety through infection-control practices and reporting unsafe conditions
- Attend in-service programs to meet compliance requirements and notify supervisors of educational needs
- Provide personal care services under the direction of the Registered Nurse or Therapist
- Support children and adults who are aging in place, medically complex, or living with chronic conditions
Requirements
What you’ll need- Successful completion of a formal certification training program and/or a written skills test and competency evaluation
- Be at least eighteen (18) years of age
- Demonstrated ability to read, write, and follow a written care plan
- Good verbal, written, and interpersonal skills
- Licensed driver with an automobile insured according to state and/or Agency requirements and in good repair
- Current CPR certification
- Current TB test results
- Current CNA license
