About the role

Key responsibilities & impact
  • Provide companionship and, if authorized, transportation according to the care plan
  • Assist clients with activities of daily living, including bathing, meals, light housekeeping, and safe ambulation/transfers
  • Report changes in clients’ conditions to the supervisor
  • Document care provided and maintain accurate agency records
  • Collaborate with the client’s care team and ensure effective communication
  • Promote client safety through infection-control practices and reporting unsafe conditions
  • Attend in-service programs to meet compliance requirements and notify supervisors of educational needs
  • Provide personal care services under the direction of the Registered Nurse or Therapist
  • Support children and adults who are aging in place, medically complex, or living with chronic conditions

Requirements

What you’ll need
  • Successful completion of a formal certification training program and/or a written skills test and competency evaluation
  • Be at least eighteen (18) years of age
  • Demonstrated ability to read, write, and follow a written care plan
  • Good verbal, written, and interpersonal skills
  • Licensed driver with an automobile insured according to state and/or Agency requirements and in good repair
  • Current CPR certification
  • Current TB test results
  • Current CNA license
