About the role
Key responsibilities & impact- Conduct home visits to implement care plans with clients and ensure a continued high level of care.
- Train and support caregivers in meeting their loved ones' needs.
- Monitor and report on clients’ health, coordinating with their care team and outside agencies as needed.
- Facilitate smooth transitions and ongoing care for clients.
- Maintain compliance with all relevant regulations and standards.
Requirements
What you’ll need- Bachelor’s degree required; preferably in social work, human services, or a related field.
- LSW or LISW in state of employment preferred.
- At least one (1) year of experience working with elders or adults with disabilities.
- Ability to travel (by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required.
- Experience in case management, care implementation, planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting.
- Training, evaluating, and supervising caregivers.
- Documenting in an accurate and timely manner; maintain a clinical record for each patient.
- Proficiency in common software and web-based applications (e.g. MS Office, case management systems, EVV, etc.).
- Patient-centered approach with strong communication skills.
- Familiarity with New Hampshire Medicaid, CFI and/or caregiving ecosystem (including CMAs, government offices, and other stakeholders) is preferred.
