About the role

Key responsibilities & impact

Screen individuals and families for social drivers of health needs and connect them with appropriate resources.
Provide referrals and linkage to community services including food, transportation, financial assistance, and other support programs.
Assist with enrollment in eligible benefits, programs, and services.
Support outreach activities and follow-up with individuals regarding resource needs.
Provide digital navigation assistance and help individuals access available services.
Complete documentation, data entry, and program tracking activities.

Requirements

What you’ll need

High school diploma or equivalent
Strong verbal and written communication skills
Current CPR Certification
Ability to work with diverse populations
U.S. citizen, national, or lawful permanent resident
Ability to meet AmeriCorps background check requirements
Preferred: Experience in community health, social services, or related fields
Preferred: Intermediate Microsoft Office skills
Preferred: Experience with electronic health record systems
Preferred: Bilingual English/Spanish skills.
