About the role
Key responsibilities & impact- Screen individuals and families for social drivers of health needs and connect them with appropriate resources.
- Provide referrals and linkage to community services including food, transportation, financial assistance, and other support programs.
- Assist with enrollment in eligible benefits, programs, and services.
- Support outreach activities and follow-up with individuals regarding resource needs.
- Provide digital navigation assistance and help individuals access available services.
- Complete documentation, data entry, and program tracking activities.
Requirements
What you’ll need- High school diploma or equivalent.
- Strong verbal and written communication skills.
- Ability to work with diverse populations.
- U.S. citizen, national, or lawful permanent resident.
- Ability to meet AmeriCorps background check requirements.
- Preferred: Experience in community health, social services, or related fields.
- Intermediate Microsoft Office skills.
- Experience with electronic health record systems.
- Bilingual English/Spanish skills.
