About the role

Key responsibilities & impact
  • Screen Clients for needs around social drivers of health and connect to internal and external resources
  • Take on short-term agency initiatives related to population health outreach, as they arise
  • Link to and/or enroll patients in services, benefits, and programs for which they are eligible (pharmaceutical programs, LINK, financial applications for hospital referrals, etc.)
  • Assist patients in digital navigation, including MyChart enrollment, setting up telehealth capabilities, and other digital assistance related to health
  • Conduct health outreach activities, through a variety of methods including campaign texting, appointment recalls, 1on1 patient phone calls, bulk referral ordering, and MyChart, as well as others
  • Support patients with scheduling specialty referrals with outside hospital partners
  • Provide linkage to services such as emergency financial assistance, transportation, food assistance, etc.

Requirements

What you’ll need
  • High School diploma or equivalent required
  • Associates degree, some college or bachelor’s degree a plus
  • Bilingual in fluency (English/Spanish) a plus
  • Verbal and written communications skills, including high level of comfort presenting to groups
  • A minimum of two years’ experience in community health, social or community services, or comparable experience preferred
  • Intermediate proficiency in Microsoft Office
