About the role

Key responsibilities & impact

Screen Clients for needs around social drivers of health and connect to internal and external resources
Take on short-term agency initiatives related to population health outreach, as they arise
Link to and/or enroll patients in services, benefits, and programs for which they are eligible (pharmaceutical programs, LINK, financial applications for hospital referrals, etc.)
Assist patients in digital navigation, including MyChart enrollment, setting up telehealth capabilities, and other digital assistance related to health
Conduct health outreach activities, through a variety of methods including campaign texting, appointment recalls, 1on1 patient phone calls, bulk referral ordering, and MyChart, as well as others
Support patients with scheduling specialty referrals with outside hospital partners
Provide linkage to services such as emergency financial assistance, transportation, food assistance, etc.

Requirements

What you’ll need

High School diploma or equivalent required
Associates degree, some college or bachelor’s degree a plus
Bilingual in fluency (English/Spanish) a plus
Verbal and written communications skills, including high level of comfort presenting to groups
A minimum of two years’ experience in community health, social or community services, or comparable experience preferred
Intermediate proficiency in Microsoft Office
