About the role

Key responsibilities & impact

Provide in-home and virtual remote palliative care to patients in the Grand Rapids area
Collaborate with company physicians, patients’ other physicians and providers, and family members
Develop complex plans of care aligned with patients’ health status, goals, and values
Provide clinical and non-clinical support to palliative-care patients within a designated territory
Conduct home visits and provide care in home settings
Develop and implement clinical plans of care for adults with chronic and complex conditions
Gather patient histories, perform physical exams, and obtain diagnostics as needed
Develop and implement treatment plans based on patients’ goals of care and current conditions
Address patients’ and families’ physical and psychosocial needs with interdisciplinary-team support
Educate patients and families about medications, side effects, illness progression, diet, nutrition, adherence, and crisis prevention
Coordinate care and resources with clinical team members, physicians, and medical providers
Maintain patient medical records and documentation in accordance with regulations and company standards
Participate in required continuing education
Prescribe medication as permitted by state prescribing authority

Requirements

What you’ll need

Michigan-licensed Nurse Practitioner
Master of Science in Nursing (MSN)
Current unrestricted Nurse Practitioner (NP) license or Physician Assistant (PA) license in applicable state(s)
DEA registration or eligibility
CPR certification
Satisfactory completion of a Tuberculosis test
Ability to provide in-home and virtual remote palliative care in the Grand Rapids area
Ability to travel; travel may be required
2 years of experience in home-based services, complex care cases, and hospice and/or palliative care preferred
Bedside hospital, oncology, emergency room (ER), or intensive care unit (ICU) experience highly preferred
Experience with Electronic Medical Records (EMR) preferred
Bilingual in English/Spanish preferred
